Your dental records are covered separately
Once you are our patient, your dental and health records are protected health information. They are handled under federal privacy law (HIPAA) and our Notice of Privacy Practices, which you can ask for at our front desk at any time. This website policy does not replace that notice.
Please don’t use the website form to send medical history, X-rays, insurance ID numbers or other sensitive details. A short note about what you need is enough — we’ll take the rest by phone at (305) 705-2748 or at your visit.
Information you give us
When you request an appointment, the form asks for:
- your name, email address and phone number;
- whether you are a new patient and whether you have dental insurance;
- a short message about how we can help.
We use this only to contact you about your request and to schedule your visit. Each request is emailed to our office (our website management company’s Google email service sends it) and a copy is kept in our website’s database. If you call or email us, we use what you tell us the same way.
Information collected automatically
Like most websites, our web host and our security firewall record technical details of each visit — such as your IP address, browser type, the pages you request and the time — to keep the site running and protect it from attacks.
We also use the services below, which can set cookies or similar technology in your browser. None of them receives your name, email, phone number or the message you type in our form.
